PERCUTANEOUS ENDOSCOPIC GASTROSTOMY (PEG)

PEG tube placement:

The patient undergoes general anesthesia and upper endoscopy. After the stomach is fully inflated,  the abdominal wall is transilluminated with the scope and a needlepercutaneoulsly inserted into the stomach;   a guidewire is introduced through the needle, grasped by a forceps from the  endoscope's instrument channel, and retrieved through the mouth. It is attached to a tapered tube with a cross bar at the end. This is then pulled into the stomach, so that the tube exits through a small incision at the level of the guidewire. The end of the tube is the cut and an adapter is fitted to keep it positioned under enough tension to prevent the stomach from moving away from the abdominal wall.
