Instructions and Forms for International Students

Institutional funding consists of need-based loans or need-based scholarship funds available from Alpert Medical School. International students (those holding or applying for either F-1 or J-1 visas) should complete the following forms and return them to the Financial Aid Office by April 15:

• AMS Application Form
• International FA Application
• Certification of Finances Forms (bank certification required. Note: this form is required only for first-time aid applicants)

Tax returns for the student, spouse and parents (including a certified translation) and W-2 Forms (if applicable). Parent non-filers must complete the Parent Non-Tax Filer Form. Student non-filers must complete section D of the Alpert Medical School Financial Aid Application. Parent information is required for students who wish to be considered for institutional funding.
