Instructions and Forms for U.S. Citizens
and U.S. Permanent Residents

Important note: Before completing any financial aid application forms, please determine whether you wish to be considered for institutional funding or external funding.  Institutional funding may consist of need-based loans or need-based scholarship funds available from Alpert Medical School or from campus-based, federal programs.  External funding comprises any source external to AMS, primarily federal student loans.  If you wish to be considered for institutional funding, parent information is required and considered in determining your eligibility.  If you wish to be considered for external funding, parent information is not required.

The following forms are required for U.S. citizens and permanent U.S. residents who are applying for institutional and external funding (i.e., Brown loans and scholarships, and federal student loans):

• Free Application for Federal Student Aid (FAFSA) http://www.fafsa.ed.gov/
  (parent information may be required)
• Need Access Application (parent information is required)
  > http://www.needaccess.org/
• Alpert Medical School Financial Aid Application
• Federal Tax Returns (parents' and student/spouse returns are required) and W-2 Forms or Non-Tax Filer Form for Parents (Student non-filers must complete section D of the Alpert Medical School Financial Aid Application)
• Alien Registration Card (required only for non-U.S. citizens who are permanent U.S. residents)

The following forms are required for U.S. citizens and permanent U.S. residents who are applying for external funding only (e.g., federal student loans):

• Free Application for Federal Student Aid (FAFSA) (parent information is not required) http://www.fafsa.ed.gov/
• Alpert Medical School Financial Aid Application
• Federal Tax Returns (student/spouse returns are required) and W-2 Forms
  (Student non-filers must complete section D on the Alpert Medical School Financial Aid Application)
